By filling out these forms before meeting with your attorney, you can save time and help your attorney to better serve you. General Client Information Documents Divorce - Dissolution and Annulment Documents Domestic Violence (Protective Order) Documents Adoption Documents Client Information Forms: For any proceeding please download and fill out the following: 1) Client Information Sheet -This document is our standard client information sheet that includes contact and personal information needed for court filings. For Divorce Matters: To start a divorce, dissolution or annulment proceeding, please download, print and prepare the following documents before you meet with your attorney. Bring these filled out forms to your first meeting. You must bring all of the below (unless you do not have any children from the marriage, you may exclude the child support obligation forms.) If you have any questions about any of the forms, call 801-566-1884. *NOTE: If you are seeking a 'no-contest' or 'uncontested' divorce, the Utah Courts website provides an online 'self-help' program you might want to consider. The Online Court Assistance Program (OCAP) can help you prepare divorce documents. However, if your divorce will be contested, please call 801-566-1884. 1) Client Information Sheet -This document is our standard client information sheet that includes contact and personal information needed for court filings. 2) Financial Statement -This is the standard Financial Declaration document used by the courts to compute income, expenses, assets and liabilities. 3) Certificate of Divorce, Dissolution of Marriage, or Annulment -Utah Department of Health required certificate. Fill out the pertinent information requested on this form. 4) Military Service Affidavit - You must fill out and sign this affidavit that declares whether you have or are currently serving in the military. 5) Child Support Obligation Worksheet & Required Location Information. (Download and fill out only if you have children from your marriage, or relationship.) -As required by technical amendments to welfare reform section 653(h)(2) (federal law) and U.C.A. 62A-11-103(14). You are required to submit for EACH CHILD the information requested on this form. |
